LIST OF DOCUMENTS for admission to clinical residency (residency) at the Tashkent State Dental Institute for the 2024-2025 academic year

| 3. 086 – U-form medical certificate; | Scan both sides of the reference in color and send it as a single PDF file. |
| 4. Photo 1 piece, 10x15 cm. | A color photo in "for documents" format, JPG format, quality 300 DPI. |
| 5. A document granting privileges for admission to studies in accordance with legislative documents. | Scan the document in color and send it as a single PDF file. |
| 6. Copy of Passport or ID | Scanners of Passport (pages with photo and registration)or ID (front and back) |
| 7. Enter your personal information into the file. | Enter your personal data in the sequence shown in the file and send it to the appropriate email. |

All documents should be collected in a single ZIP folder and sent to the email address klordtsdi2024@gmail.com.

If the documents are submitted correctly, a "Confirmation letter" will be sent to your e-mail.
A personal number (ID) will be sent to you in the "Confirmation Letter".
